Network Provider must furnish non-emergency treatment
to Prudent Buyer HCO enrollees within twenty four (24) hours of request. |

Emergency services are to be available twenty four (24) hours per
day, seven (7) days a week either directly by the Provider or by appropriate
physician coverage. |

Network Providers need to provide enrollees telephonic direction to access care after hours. |

Employees that do not want to be treated by the HCO after a work
related injury or illness may designate their own personal physician,
personal chiropractor or personal acupuncturist who has treated the
employee before and has their medical records. The predesignated physician
must also be willing to treat the employee for a work related injury
or disease and must be of the appropriate specialty to do so. |

Enrollees in the HCO may elect to change treating physicians during
the course of treatment or obtain second opinions by contacting Prudent
Buyer HCO at (877) 426-2583. |
